Vitalis are recruiting for experienced Qualified Social Worker to work in an Specialist Learning Disabilities ‘Positive Pathways’ Team in Manchester City Council.
We are seeking an experienced Senior Social Worker to provide maternity cover within the Positive Pathways Team.
This is a specialist team working with adults with a diagnosed Learning Disability who are transitioning back into the community after periods under Section 3 of the Mental Health Act and/or after complex prison releases.
Our work focuses on preventing hospital readmission, reducing the risk of reoffending, and stabilising individuals post-discharge or release for up to 12 months.
This is not a standard Senior Social Worker role. The team works with some of the highest‑risk individuals in Manchester, and we are frequently the lead agency in managing multi‑agency risk, statutory responsibilities, and legal processes.
The successful candidate must be able to hold complexity, provide robust professional oversight, and support staff to deliver best‑practice social work.
