 We are recruiting for a Qualified Social Worker to work in a Hospital Discharge Team in Oldham Council.  

Social worker required for the hospital discharge to assess team based at Wythenshawe hospital
Experience of working in discharge to assess and hospitals required 
 
There is plenty of hybrid working available.
