Vitalis Social Care are recruiting for an experienced and qualified Social Worker to join a hospital team in the Southwest of England.

Job Role and Responsibilities: -
We are looking for an experienced Social Worker who is committed to working as part of a multi-disciplinary team to safely discharge adults 18 years and older. Our team work mainly to the Discharge to Assess model (D2A), following cases post discharge to ensure needs are assessed once the individual is within their community or in some cases once in a temporary placement to further plan their discharge home. We also support individuals who have more complex needs that cannot be met by the current D2A services, participating in ward discharge planning meetings, Multi-agency Risk Management Meetings (MARM’s) & Best Interest Meetings.

If successful, you will be working across the whole of the BCP Council area supporting our acute hospitals, community hospitals, residential placements and clients’ homes.
There is also an expectation around supporting front of house services (A&E and assessment units) in our hospitals to prevent unnecessary admissions to hospital.

You will join a highly motivated and supportive team and will be encouraged to engage in reflective supervision, peer support and undertake appropriate and relevant training.
