Adult Social Worker, Discharge to Access Team, Tower Hamlets  

Social Worker required for Discharge to Assess in Initial Assessment. Discharge to assess supports residents who have been discharged from the hospital with a package of care, where they previously did not have statutory support in place. To complete care and support assessments, working with residents, their support networks and other professionals. Essential: experience of at least 2 years in Adult Social Care Rolling contract, 3 months initially 

Pay - £32.53 per hour
