Job Description

Senior Social Worker, Adults South Community Hospital,  Staffordshire 
 
Location: South Community Hospitals (Samuel Johnson Community, Sir Robert Peel), Burntwood, Lichfield, Tamworth, East Staffs, and surrounding areas 
Social Worker within the South Community Hospital Discharge teams. The primary responsibilities include facilitating timely patient discharges and preventing delayed transfers of care for Staffordshire patients placed in Discharge to Assess beds (D2A). Unplanned Care South East manages the D2A service in Burntwood, Lichfield, Tamworth (BLT), East Staffs, and nearby areas, working alongside Multi-Disciplinary team members such as Social Workers, Social Care Assessors, Nurses, Therapists (Occupational Therapists and Physiotherapists), and the MPFT Home First Team. The role involves collaboration with Multi-Disciplinary Teams across various hospital and community settings. 
Applicants are required to have experience with the Care Act (2014), Mental Capacity Act (2015), and conducting section 42 enquiries. The role requires effective communication and IT skills, along with the ability to work independently and as part of a team, sometimes under pressure. Duties include completing Statutory Social Care Assessments and support plans, Mental Capacity Assessments, Best Interest Meetings, Deputyship Referrals, and participation in Continuing Healthcare Decision Support Tool Meetings.
