Occupational Therapist – Reablement at Home Pilot
Location: Hackney
Rate per hour: £34-£6 per hour
Work type: Community Based
Contract: Full Time Contract

We are currently recruiting for an experienced Occupational Therapist to join an exciting new Reablement at Home Pilot Service within London Borough of Hackney Adult Social Care.

This innovative service has been designed to help residents regain independence in their own homes through a therapy-led reablement approach, reducing long-term care needs and supporting safer hospital discharge.

You will work as part of a specialist reablement team supporting adults in the community by:

• Completing functional assessments in service users’ homes
• Developing person-centred reablement plans
• Setting realistic therapy-led goals
• Monitoring progress and reviewing outcomes
• Working closely with providers and multidisciplinary teams
• Supporting individuals to regain daily living skills
• Promoting independence and reducing reliance on ongoing care

The Service The pilot offers:

• Up to 6 weeks reablement support
• Therapy-led interventions
• Community-based working
• Strengths-based practice
• Hospital discharge support
• Improved long-term outcomes for residents

Requirements To be considered, you will need:

• Qualified Occupational Therapist
• HCPC registration
• Previous adult social care experience
• Reablement or community experience
• Strong assessment skills
• Ability to work autonomously in the community
• Full UK driving licence preferred
